California DMV Information

•  The California DMV may require you to obtain a completed Vision Report from our office.  The two-page California DMV vision report may be downloaded from the following link.  You may fill in your information on this form online and send the printed copy with a fee of $45.00 to our office for completion.  Alternatively, you may bring this form to our office and submit a fee of $45.00.  Our office staff will mail it back to you when it is completed.
